Applications are sought from experienced Registered Nurses for the Northeast Health Wangaratta Community Palliative Care Service. The Palliative Care Community Health Nurse functions autonomously and collaboratively to provide a comprehensive holistic clinical assessment and implementation of a care plan based upon a client’s health status. They will receive support, guidance and mentoring through the multidisciplinary care team.
